Foot Smother

You might want to use the tablefootnote package (and probably you want to remove the \begin{footnotesize} \end{footnotesize}, otherwise the table is printed in footnote size;

The footer logo is about 3 x 20cm. The header is shown on the correct place, but the footer is not shown at all. You can set \fancyfoot wherever you want (better after a \clearpage command, of course).
